In partnership with our health clinic we screened patients with diabetes who had a HGA1c over 7 and food insecurity for referral to healthy food center. The goal was to educate patients on healthy eating and see if diabetes could be better controlled.

Adults with diagnosis of diabetes and HGA1c over 7 (diabetes not in control).
Patients who screened positive as food insecure .
Hospital resources: Community health worker to screen patients while at doctor visit.

Referred to food bank who then distributed food to patient and family on weekly basis .

Consistent access to fresh fruits and vegetables increased over course of the program.
Fruit and vegetable intake increased. The response of “More than once a day” increased by 33%.
Individual self-reported health outcomes increased. The majority shifted from “Poor” and “Terrible” (78%) to “Very Good” and “Good” (80%).
Quality of life indicators show an improvement. A 27% increase in those who are “Never” kept from their usual activities due to poor mental or physical health.
